Tourism as a sector has contributed significantly to regional development, and its activities often bring aggregate benefits to local and national economic growth. Moreover, tourism activities require meticulous planning, particularly in the domain of regional tourism development. As a destination rich in Batak Toba culture and the natural beauty of Lake Toba, Samosir is faced with a dilemma between maximizing its tourism potential and preserving its cultural heritage. This study seeks to develop a planning strategy for sustainable tourism on Samosir Island, drawing upon local wisdom and practices. The research employs a mixed-methods approach, incorporating Participatory Rural Appraisal (PRA) alongside quantitative analysis through Smart PLS, with a sample size of 100 respondents. The study's findings indicate that local values can be effectively integrated into regional development planning for tourism areas, thereby fostering sustainable tourism and promoting adaptive, inclusive policies. However, the integration of local wisdom into tourism policies remains challenged by structural issues, including low institutional capacity and the lack of a fully developed framework, which requires a strategic approach to regional development planning in tourism areas that leverage their local values. According to the quantitative results, regional development planning has a significant positive effect on sustainable tourism (β = 0.407; p = 0.000), local wisdom shows a strong role in supporting sustainable tourism (β = 0.571; p < 0.001), and tourist areas have a large effect on local wisdom (β = 0.658; p < 0.001) but the direct effect of tourist areas on sustainability is not significant (β = –0.047).
